Patient Portal
Your patient portal is the easiest way to access important information related to your care at Unity Counseling.
Through the patient portal, you can:
- Schedule Appointments: Existing clients can request new appointments, or cancel and reschedule existing ones (subject to clinician approval).
- Complete and Sign Paperwork: Fill out electronic intake forms, custom questionnaires, and client history forms online, and sign documents electronically. Clients can also download completed versions of these forms.
- Make Payments: Pay balances securely using a credit, debit, or HSA card, and view or download payment receipts and history.
- View Shared Files: Access documents, educational materials, or outcome measures that your provider has made available or marked as "always available" on your portal.
- Use Secure Messaging: Communicate safely with your provider through the integrated messaging system.
Need Help With the Portal?
Your email address on file is your Patient Portal ID. If you’ve forgotten your password or are having trouble logging in, use the Forgot Password option on the portal login page to reset it.
If you need to update the email address associated with your account, contact your provider for assistance.
